  1. Transcribe line items; do not upload a full chart (5 min).

    Copy provider name, date of service, billed amount, allowed amount, what you paid, CPT/HCPCS/Rev codes as printed, and why you dispute (duplicate, wrong date, not received, already paid). Prefer last four of member ID. Omit SSN and other family members’ claims.

  2. Lock “verify codes / no invented lines.”

    The model may not add a CPT or a law citation you did not supply.

    Every code in the letter must appear on your statement. If the model added one, delete it.
